Registration of your 'Chipped Quilt in the National Quilt Registry is the most important step in identifying your quilt. Without registration, we have far fewer resources when it comes to keeping you and your quilts together. Please take the time to register each quilt that you have 'Chipped.

Print out the Registration Form (PDF format) and either fax or mail in your registration with your check payment of $12.00 for single registrations (please refer to registration form for payment if submitting multiple registrations).
